CORI Form (Annually)
It is mandated by Massachusetts state law that all NCC staff and volunteers submit a Criminal Offender Record Information (CORI) background check each year to ensure the safety of children, youth, elderly, and the disabled. Since it is necessary for you to show a photo identification (license), CORI forms have to be completed in person at the Parish Office or with a member of the NCC staff.
A completed CORI is required for all new volunteers before beginning service, and CORI checks are repeated annually for all volunteers, usually in the spring.
Please Note: You can not start volunteering until the CORI has come back to the office cleared.
To complete the form, your government-issued ID must be checked by a NCC employee. The staff person who coordinates your volunteer service may arrange a time for this, or you can come in to the parish office during regular business hours. Alternatively, you may bring your ID and the form to a Notary Public who will verify your identification and sign and seal the form, after which it can be returned to the parish office.
The Archdiocese of Boston issues a new CORI form each year. The form for the current year is below for your convienience.
VIRTUS Protecting God’s Children (One-Time)
All volunteers and staff in the Archdiocese of Boston, including those who do not work directly with children’s programs, are required to complete the VIRTUS – Protecting God’s Children training. New volunteers must take this 3hr training before volunteering so that they are in full compliance with the requirements of the Archdiocese of Boston and our NCC Collaborative.
In-person sessions are offered periodically at our Collaborative as well as at locations throughout the Archdiocese of Boston.

If You Attend a VIRTUS Session at Another Location:
After completing your Virtus training at another location or online, please email Mr. William Sawyer at [email protected] so he can make sure it is recorded correctly with the Archdiocese and document it in our records.
If You Completed VIRTUS Training in the Past:
If you completed VIRTUS training at another Archdiocese of Boston parish or school in the past you do not have to take it again. The Archdiocese of Boston will have a record of your completing the training. Please email Mr. William Sawyer [email protected] with the information so he can locate the record and document it in our records.
